Google Form import

Import client responses from a Google Form by downloading the linked Responses Sheet as a CSV. Imported responses become the client’s latest intake and can be used to prefill LOEs.

Go to Intake
Step A — Export CSV
Google Forms → Responses → Link to Sheets → in Sheets: File → Download → CSV
  1. Open your Google Form.
  2. Go to Responses and click Link to Sheets.
  3. Open the sheet and download the Responses tab as CSV.
  4. Upload the CSV below.

Step B — Map columns
Map each Google Sheet column header to an intake field (used for LOE prefill).
Tip: you can add extra fields by editing the intake form JSON under Intake → Form configuration, then come back here to map new fields.
